District Profiles

The Office for Health Improvement & Disparities (OHID) provides health profiles for each local authority in England. These profiles contain data on a range of indicators for local populations, highlighting issues that can affect health in each locality.

The profiles are intended to help local government and health services make plans to improve the health of their local population and reduce health inequalities.

You can search here by postcode, town, or local authority to find the relevant profile. 

Extra Care Profiles

Extra Care Profiles contain a selection of information at a local level; Population Demographics and Projections; Health; Disability;Deprivation; Housing and Care Home sites.

Lincolnshire Digital Health Toolkit

The Lincolnshire Digital Health Toolkit is a tool, developed by Lincolnshire County Council’s Public Health Team, to highlight digital exclusion in Lincolnshire.

Neighbourhood Team Area Profiles

Neighbourhood Team Area Profiles contain a wealth of information relating to Geographical areas within Lincolnshire and the people who are accessing GP services; Patient Demographics, Deprivation, Burden of Disease, Disease Prevalence, Hospital Admissions – Inequalities, Mortality, Prescribing, Access to Services, and Wider Determinants of Health.

Primary Care Network Profiles

Primary Care Networks (PCNs) are groups of GPs working together to deliver primary health services for patients. The profiles below provide health and demographic information on the 14 PCNs within Lincolnshire. They include data on:

  • Extent and distribution of deprivation
  • Burden of disease
  • Population structure
  • Projected changes to population structure
  • Diagnoses of key long-term diseases
  • Accessibility to GP services
  • Mortality rates and life expectancy
  • Wider determinants of health

The aim is to help organisations across Lincolnshire’s Integrated Care System better understand the needs of their populations and support efforts to improve outcomes and reduce inequalities.
